When you make your first appointment,
you will need to fill out my Intake Packet which includes a:

(1) Client Consent to Treatment Form
(2) Client Personal Information Form
(3) HIPPA Confidentiality Form
(4) Acknowledgement of receiving HIPPA (requires your signature)

You may download the Intake Packet from my web site, complete it and bring it with you, or I can have it available for you to complete in my office prior to our session.  If you choose to fill out the forms in my office, please arrive about 10 minutes early to do so.  Please let me know so I will leave the paperwork out for you in the waiting room.
